Carolina Complete Psychiatry is currently accepting new patients over the age of 5. Please call us to schedule your first appointment. Prior to your arrival, we ask that you complete the form below.

At any time, you may submit an update to your payment method or a release of information. This is used to be able to communicate with other treatment providers, such as your therapist or other medical providers.

Prefer Filling Out The Forms In Person?

If you would prefer to have paperwork mailed to your residence, we are happy to accommodate this request if your appointment is booked for more than 7 days from the time of request.

If you would prefer to complete these forms in the office, please arrive approximately 30-45 minutes to your scheduled appointment time and you will be provided with a computer to complete the online forms.

Self-Pay Disclaimer

Please note that Carolina Complete Psychiatry is a self-pay office so that we can provide quality care in a very timely manner. This allows us to focus our work directly with our patients without restrictions or delay.

When booking a new patient appointment, a credit card number will be needed to hold your appointment time. At the time of your appointment, payment will be accepted by cash, check, debit or most major credit cards.

Please refer to our Office Policies listed above for more information on our fee schedule, including policies regarding no-show appointments or late arrivals.

We will happily provide a billing slip that can be sent to your insurance company for direct reimbursement for those with “out of network benefits”. It is each patient’s responsibility to know their own insurance requirements, but we will gladly help as we are able.
